General Information
We know that personalised, well‐planned support services (“reablement”) for up to six weeks can not only accelerate discharge rates, but reduce care dependency and hence prevent or delay the need for long‐term home care and other labour‐intensive community services. Remote care as part of a Hybrid care model can play an important enabling role – managing the risks, both during the reablement period, and in the months following service delivery, irrespective of whether the individual needs long‐term home care.
