Remote care pathway for discharging patients (reablement)

You will learn how to empower independence and the route to proactive and preventative care

Who is this for?

  • Local Authorities and Councils
  • Care and Support Providers

General Information

We know that personalised, well‐planned support services (“reablement”) for up to six weeks can not only accelerate discharge rates, but reduce care dependency and hence prevent or delay the need for long‐term home care and other labour‐intensive community services. Remote care as part of a Hybrid care model can play an important enabling role – managing the risks, both during the reablement period, and in the months following service delivery, irrespective of whether the individual needs long‐term home care.

GenieConnect® aims to maintain and/or improve an individual’s safety and independence, adding multiple touch points to enhance overall wellbeing, and prevent or reduce the risk of secondary health conditions.
